The affidavits below are "fillable." You can fill in most of the information and print the document in order to have it notarized. You will not be able to save the contents of the completed form. You will only be able to print it. Affidavits must be notarized by a certified notary.

Ineligible for Service. Please send your documentation to Jury Services at info.juryservices@fultoncountyga.gov

Not a resident of Fulton County at the time of service: Requires documentation of new residence. We accept a driver’s license showing change of residence; agreements in the juror’s name in the county of new residence; a utility bill in the juror’s name with the new address listed (i.e., water bill; sewer bill; electric; gas; phone landline. We do not accept cell phone bills).
Not a United States citizen: Requires documentation such as a Permanent Resident card or a passport showing sole citizenship in the juror’s country of origin.

Permanent Disqualification (Requires Notarized Affidavit)

*These affidavits are permanent exemptions.

FORM: Age 70 and older
FORM: Permanent mental or physical disability

Temporary Excusals or Deferrals (Notarized Affidavit is required for each summons received)

*These affidavits are good for 12 months. You must resubmit the next time you are summoned for jury duty.

FORM: Convicted felon whose rights have not been restored
FORM: Caregiver for Children under 6 years old
FORM: Sole caregiver for someone over 6 years old
FORM: Home School Teacher
FORM: Full-time student (12 credit hours or more)

I Cannot Make Jury Duty for Other Reasons

If you feel that you have a reason, other than the approved exemptions, that you cannot make your jury duty, please call us at (404) 612-4600, Monday-Friday from 10:00 AM-3:30 PM.
